Competition season is almost here, and that means it's time to fill out permission slips for each bus trip your student(s) will take with their choir(s). Students will NOT be allowed on these trips without a signed permission slip on file.
You may choose to either e-sign your permission slip packet OR to download, print, and complete by hand for your student to return in person to Mr. O'Connell. Do this by clicking on the appropriate link under your student's choir name, below.

Through 2/14/26, ShowStars, ShowStoppers, and ShowMen families (including those with students also in ShowCase), may choose to fill out the Permission Slip Information form for your student(s) on your open forms page (CLICK HERE). You will then be emailed a mostly-completed packet to e-sign on 2/15/16.
